Nurse's Corner

I am excited to welcome you into the building for the 2022-2023 school year! I am looking forward to working together to assure a safe and healthy year! The goal of the School Nurse is to support the educational process by promoting optimum wellness and working closely with scholars, parents/guardians, teachers and administrators; assess and evaluate the health need of scholars; and to accommodate scholars with specific health concerns.

Medical Requirements

Each year our students are placed into class advisories where they work closely with their advisors and fellow classmates. Spending half the day in these advisories, students develop deep connections as they collaborate on projects that challenge them to think critically and develop new skills.

All Students

  • Form S865:
    Required for all students
    Completed by a parent/guardian
    To be submitted every year and if health status changes an updated form is required
    MUST be on file for student to receive acetaminophen (Tylenol/Tylenol Extra Strength) or ibuprofen (Advil/Motrin) in over the counter dosages

  • Form MED-1:
    Must be completed by student's medical provider 
    Required for administration of acetaminophen or ibuprofen greater than the over the counter dose
    Required for any other medication while at school
    ALL PRESCRIPTION MEDICATIONS REQUIRE THIS FORM

    • ​Insulin pumps, rescue inhalers and epi-pens may be carried with the student as long as an MED-1 is on file with the nurse.  All other medications must be taken to the nurse in a pharmacy container along with proper documentation.

New Admits
(9th Grade)

  • Student Emergency/Medical Information form S865 completed by a parent/guardian

  • Proof of required immunizations before being admitted to school

  • Students who are new to the School District ONLY are required to provide proof of a physical exam via a Physical Examination Form (MEH-1) completed by a medical provider

  • Medical exemptions for immunizations must be in writing by the student’s provider

  • Religious exemptions must be in writing by  the parent

  • Prescription medications require a Request for the Administration of Medication in School form (MED-1) completed by student's provider

  • Acetaminophen (Tylenol/Tylenol Extra Strength) or ibuprofen in over the counter amounts require an S865 on file in the nurses' office

  • Acetaminophen (Tylenol/Tylenol Extra Strength) or ibuprofen in more than over the counter amounts require a Request for the Administration of Medication in School form (MED-1)
     

11th Grade

  • Rising Juniors at The Workshop School need to submit updated proof of a comprehensive physical exam (well-child check-up) to be in compliance with this change. Please have your child’s medical provider complete and sign a Report of Physical Examination dated after July 1st after their sophomore year to be submitted to the school nurse at the beginning of the school year. Forms dated prior to July 1st may be returned to the student.

12th Grade

  • Students entering 12th grade or at the age of 18 years are required to provide proof of a second meningococcal vaccine (MCV). This vaccine is also known as Menactra or Menveo. Please submit proof of vaccination or exemption to the CSN at the start of 12th grade.
